Do you have a plan for your end-of-life care?

As I approach the 'older than dirt' age, I think about when and how I'd like to die. I've made out a health care directive (a living will), and a Medical Power of Attorney specifying a decision maker and the medical care I want/don't want towards the end of life. I want quality, not quantity, of life, plenty of pain meds and classical music.

I think that all of us oldsters should be making plans about such stuff. Doctors should not be the primary decision makers. Their role should be giving options and answering the important questions:

What are the benefits and side effects from a given treatment? ...
